[18F]Fluoro-RS-86 is a radiopharmaceutical and Positron Emission Tomography (PET) imaging agent designed to visualize and quantify muscarinic acetylcholine receptors (mAChRs) in the brain. It is a fluorinated analog of RS-86, a muscarinic agonist originally developed by Sandoz for the treatment of cognitive deficits in Alzheimer's disease. As a PET tracer, [18F]Fluoro-RS-86 binds to muscarinic receptor subtypes (M1 through M5), allowing for the non-invasive assessment of cholinergic system integrity in vivo. While the parent compound RS-86 was noted for its functional M1 selectivity, the radiolabeled version is primarily utilized in clinical research to study neurodegenerative disorders characterized by muscarinic receptor loss or dysfunction. The tracer provides critical insights into the pathophysiology of Alzheimer's disease and other dementias, although it has been largely superseded in modern research by tracers with higher subtype specificity or improved pharmacokinetic profiles.
